In order to participate in MCPS athletics programs, students are required to complete a Pre-Participation Physical Evaluation for Athletics (SR-8) form. The document has been linked below for your convenience:
Pre-Participation Physical Examination Form
The Pre-participation Physical for Athletics form must be completed and submitted before a student-athlete can participate. Parents/guardians should scan and upload their student's physical, when completing the registration process.
Students enrolled in grades 7 & 8 must have a medical evaluation completed every two years in order to participate in MCPS middle school interscholastic athletics program. This means that the Medical Eligibility Form will be valid for two-years from the date the exam and covers participation for any athletic program held within that two-year time frame.
PLEASE NOTE: Your child's Pre-Participation Physical Evaluation for Athletics (SR-8) Form MUST be STAMPED and signed by the health care provider who completed the Medical Eligibility Form.
An example of a completed Medical Eligibility Form with a certified doctors stamp has been included below:
If your insurance provider is KAISER PERMANENTE you will not submit MCPS' SR-8 Form. You will instead submit their Physical Examination Certification Form. Please request this document if is not provided to you at the end of the appointment.
