Medical Evaluation

Pre-Participation Physical Evaluation for Athletics (SR-8 Form)

In order to participate in MCPS athletics programs, students are required to complete a Pre-Participation Physical Evaluation for Athletics (SR-8) form. The document has been linked below for your convenience:

  • The student's medical evaluation must be performed by an authorized health care provider
  • The Pre-Participation Physical Evaluation consists of four parts: History Form (pg. 1 & 2), Physical Examination Form (pg. 3), Athletes with Disabilities Form (pg. 4), and Medical Eligibility Form (pg. 5). Pages 
  • Once complete, the Medical Eligibility Form (pg. 5) should be submitted (uploaded) online via ParentVUE Portal. The authorized health care provider should retain the first four pages of the Pre-Participation Physical Evaluation For Athletics Form (pg. 1-4)

The Pre-participation Physical for Athletics form must be completed and submitted before a student-athlete can participate. Parents/guardians should scan and upload their student's physical, when completing the registration process.

Students enrolled in grades 7 & 8 must have a medical evaluation completed every two years in order to participate in MCPS middle school interscholastic athletics program. This means that the Medical Eligibility Form will be valid for two-years from the date the exam and  covers participation for any athletic program held within that two-year time frame.

PLEASE NOTE: Your child's Pre-Participation Physical Evaluation for Athletics (SR-8) Form MUST be STAMPED and signed by the health care provider who completed the Medical Eligibility Form.
